To request a copy of your LCS transcript, fill out the form below.
- Fill out one form per institution you wish to have your transcript forwarded to.
- Transcripts will be forwarded directly to the requesting institution using the information provided, unless otherwise specified in the last field.
- Please note, transcripts issued directly to the student will be marked as such and may not be considered an official transcript by the receiving institution.
